The scoring is honest by design: the toolkit reports a Quality Culture Index — but the index is capped by your weakest aspect, and your overall level is set by that weakest aspect, never by the average. A strong paperwork culture cannot hide weak behaviour: the headline number can never outrun the culture you actually have. No aspect is judged on a single source of evidence — survey, focus groups and operating indicators are triangulated before any level is assigned.
